On or about November 16, 2006, while employed in a hospital in Houston, the RN falsely documented the physical assessment of a patient. She documented receiving the patient in the unit, transferring the patient to the bed and making the patient comfortable at 18:50 pm, when in fact the patient did not arrive in the unit until 20:30 pm, after her shift had ended.

She explains that she went to the Emergency Department to interview the patient before the patient actually arrived on the unit because there were several admissions to the unit and she was trying to help expedite the admission process. The patient at that time appeared to be stable and in no apparent distress. According to the RN, the patient information she received was from family members and not from the patient. No harm occurred to the patient as a result of any inaccurate medical record entries.
However, the Board states that her conduct resulted in an inaccurate medical record which created the potential to injure the patient in that subsequent care givers would not have reliable information on which to base their care decisions.
