At the time of the initial incident, an RN was employed as a Registered Nurse at a hospital facility in Frisco, Texas, and had been in that position for five (5) months.

On or about July 29, 2016, while employed as a Registered Nurse and caring for a Patient, the RN failed to document a complete initial set of vital signs including heart rate or respiration rate. The RN’s conduct resulted in an incomplete medical record and could have contributed to the injury of the patient in that subsequent caregivers did not have accurate and complete information on which to base their decisions for further care.

On or about August 18, 2016, while employed as a Registered Nurse and caring for a Patient, the RN failed to document a complete initial set of vital signs including heart rate or blood pressure. The RN’s conduct resulted in an incomplete medical record and could have contributed to the injury of the patient in that subsequent caregivers did not have accurate and complete information on which to base their decisions for further care.

In response to the incident, the RN states that initial vital signs were performed and documented on 7/29/16 at 21:15. The RN states that vital signs and assessment were recorded when he realized that the Patient Care Technician had not completed vital signs prior to the documentation of his assessment. 

In addition, the RN states that vital signs for temperature and respiratory rate were noted at 20:19; however, there is no blood pressure notation. The RN states that the blood pressure noted by the nurse on the prior shift was tagged as high, but the nurse noted that the baby was kicking and therefore the blood pressure was not reliable. The RN states that he was at the bedside with the patient obtaining a complete assessment, including temperature and respiratory rate. The RN states that the reason a blood pressure would not have been taken was due to the baby kicking and being upset/active, such that an accurate blood pressure reading could not have been obtained.

The RN was summoned by the Texas Board of Nursing to explain her side. However, the RN failed to hire a nurse attorney to help her with her case. Because of this, the Texas Board of Nursing suspended and disciplined the LVN. The Board stated that by failing to take appropriate actions, the LVN allowed a potentially life-threatening condition to go untreated.
