On or about July 28, 2019, while employed as a Licensed Vocational Nurse in a medical facility in Lewisville, the LVN falsely documented the administration of Vitamin A and D ointment to a patient in that she left her assignment at 1241 and did not return. Her conduct was likely to harm the patient in that subsequent caregivers would rely on her documentation to base their decision to administer further care.

Because of the incident, the Texas Board of Nursing summoned the LVN to hear her side of the story. However, the LVN failed to appear at the hearing. Because of this, the Texas Board of Nursing disciplined and suspended the LVN.
