At the time of the initial incident, the RN was employed at a hospital in Amarillo, Texas, and had been in that position for one (1) month.

On or about October 31, 2016, through December 30, 2016, the RN failed to submit completed nursing documentation for the patient. The RN’s conduct resulted in an incomplete medical record and was likely to injure the patient from subsequent care decisions made without the benefit of accurate and reliable information.

On or about November 10, 2016, through November 16, 2016, the RN failed to submit completed nursing documentation for the patient. The RN’s conduct resulted in an incomplete medical record and was likely to injure the patient from subsequent care decisions made without the benefit of accurate and reliable information.

And on or about November 22, 2016, through December 28, 2016, the RN failed to submit completed nursing documentation for a patient. The RN’s conduct resulted in an incomplete medical record and was likely to injure the patient from subsequent care decisions made without the benefit of accurate and reliable information.

Then on or about November 29, 2016, through December 26, 2016, the RN failed to submit completed nursing documentation for care rendered to the patient. The RN’s conduct resulted in an incomplete medical record and was likely to injure the patient from subsequent care decisions made without the benefit of accurate and reliable information.

In response to the above incidents, the RN states that while working at the hospital, she would document on a notebook during her home visits. She would then go to the Library to document the patient charts after her visits. And that when she left the company they informed her that she had missing notes that needed to be completed. The RN reports that she spoke with staff who would review the documentation and she informed the RN of what was missing. The RN states that she informed her that she would complete the notes. She also states that when she tried to log into the computer system it would not allow her to log on. Then she contacted the owner and got a new username and password, then completed the notes. The RN states that she did not complete notes for days that she did not see the patients and notified the office that she did not see the patients on those dates. The RN states that the office contacted her on those dates and informed her that the RN would see the patient instead. The RN states that to this day all of her visit notes are complete.

As a result, the RN is subjected to disciplinary action by the Texas BON. The decision was based on the evidence received by the Board which led to the RN’s case. . If the RN will not comply, his/her license will be put at risk. To prevent things such as suspension or revocation, it is a must to have a nurse attorney for support or for defense.
