On or about July 21, 2013, while employed as a Home Health Nurse in a medical facility in McAllen, the LVN mistakenly stocked the pill box of a patient so that the patient would take Coumadin 5mg twice daily instead of once daily, as prescribed.

Two (2) days later, the patient was transported to a local emergency care center due to continued shortness of breath, left foot edema, and dry cough, at which time it was discovered through lab tests that the patient had been receiving too much Coumadin.

The patient was discharged to home in stable condition after being hospitalized for three (3) days. The LVN’s conduct was likely to injure the patient from adverse complications of Coumadin overdosage, including possible uncontrollable bleeding and/or demise.

In response to the incident, the LVN states that, although it is not clear whether or not the medications were tampered with or removed or added/reloaded by another party, if the LVN made the error it is possible that she looked at the wrong medication.

However, without the valid reason and defense to properly explain the LVN’s side, the LVN was disciplined and suspended by the Texas Board of Nursing.

Because of this incident, the Texas Board of Nursing then subjected the RN’s license into disciplinary action.
