An LVN was employed as a Licensed Vocational Nurse at a medical facility in Victoria, Texas, and had been in that position for three (3) years and three (3) months.

On or about October 12, 2018, while employed as a Licensed Vocational Nurse at a hospital in Mesquite, Texas, the LVN inaccurately documented two different weights of the patient in that the LVN documented weight of 193 lbs at 13:40 and 200 lbs at 14:14. The LVN’s conduct resulted in an inaccurate medical record and was likely to injure the patient in that subsequent caregivers did not have accurate information on which to base their decisions for further care.

On or about February 7, 2019, while employed as a Licensed Vocational Nurse, the LVN inaccurately documented an assessment on a patient for an office visit that occurred on September 14, 2018, and failed to indicate that it was a late entry. In addition, the LVN also inaccurately documented an assessment on the patient for an office visit that occurred on September 21, 2018, and failed to indicate that it was a late entry. The LVN’s conduct resulted in inaccurate medical records and was likely to injure the patients in that subsequent caregivers did not have accurate and complete information on which to base their decisions for further care.

In response to the incident, the LVN states that she corrected items and added notes to the files.

However, the LVN was not able to provide a good defense for himself, especially when the evidence was brought up to him. Therefore, the Texas Board of Nursing (BON) placed her LVN license to disciplinary action instead.
