On or about January 10, 2007, after leaving employment at a medical facility in Bryan, Texas, the LVN returned to the facility and falsely documented twenty-four (24) Nurse Communication Notes in the medical records of patients, as documentation for twenty-three (23) missed visits and one (1) abnormality visit. The Nurse Communication Notes were dated between August 21, 2006, and September 28, 2006, and each indicated that LVN had notified the RN, of the missed visit or patient abnormality. The aforementioned RN was not employed by the facility until September 26, 2006. The LVN’s conduct was deceptive and resulted in inaccurate medical records. 

In response to a said incident that happened, the LVN states that she was coerced and threatened by the facility’s administrator to provide the missed visit notes or be reported to the Board for failure to provide documentation. According to LVN, on that date, she and other employees were all called to the facility and instructed to put another RN as the person notified because the person who actually was contacted was no longer employed at the facility. The LVN continues, indicating that she and the other employees were each given “stacks” of records with missing documentation and they were instructed to “just fill in the forms so the charts would be complete” for an upcoming audit.

Because of this, the Texas Board of Nursing found the LVN’s conduct inappropriate. Thus, the LVN was found guilty and her license was disciplined and suspended.
