On or about August 1, 2018, while employed as a Registered Nurse, the said RN falsely documented receiving a telephone order for a prescription of Diazepam (Diastat) for a patient from the patient’s physician the prescription was subsequently submitted to Killeen, Texas

As a result of the incident, the RN states she did not intend to deceive the pharmacy nor forge a prescription to possess the drug, Diazepam (Diastat). The RN, with her LVN trainee, clarified the prescription written from the previous year due to missing parameters. She mentioned that she requested her LVN in training to reach out to the physician’s clinic using the form “Physician’s Order Request/Order Clarification” which is a generic internal form. It was a miscommunication issue when the pharmacy interpreted the form as the prescription. 

After the incident and the investigation, formal charges were filed and mailed to the RN. According to the investigation made by the Texas Board of Nursing, the RN was found guilty and is subjected to disciplinary action.
